Information on appointments

Appointments system

During the height of the COVID pandemic we needed to remove the online appointment booking facility, we have now started to put appointments online to book through the NHS App or SystmOne Online. If you book via the online system you are also able to cancel the appointment without the need to call the surgery, log into the app and cancel the appointment.

Your form will be rejected by the practice if it is not supported by a form of ID

If you would like to use our on-line service please click on the application form link below:

St Agnes Surgery - The surgery is open from 08:30 to 18:30 Monday to Friday.

Mount Hawke Branch Surgery - Open 08:30 – 12:30 on Mondays, Tuesdays, Thursdays and Fridays.

Appointments can be made at the reception desk or by telephone during the hours that the surgery is open. Some appointments can be pre-booked up to eight weeks in advance with the doctor of your choice. However, all other appointments will be not be available until the day you ring the surgery, but not necessarily with the doctor of your choice.

You may choose to see any doctor in the practice, not necessarily the one whose name is on your medical card. However, it is probably better to try and see the same doctor during one episode of illness.

Urgent cases will always be seen on the same day, although we cannot guarantee that you will see the doctor of your choice. Please do not ask for urgent appointments for non-urgent cases.

Call recording

All telephone consultations (incoming and outgoing calls) are recorded for your safety, for staff training and audit purposes. These calls can only be accessed by the Practice Management and by requesting a 'Subject Access Request' form.  You will then be invited into the practice to listen to the call.  If you do not want your call recorded inform the clinician and they will rearrange for you to have a face to face consultation at another suitable time

Improving access to general practice

We are participating in a national incentive to offer evening and weekend appointments to make it easier for you to see a doctor, nurse or other healthcare professional at a time more convenient to you between 18:30 – 20:00 Monday to Friday and at the various hours at the weekend. The aim of the national scheme is to help people who may find it difficult to see a healthcare professional during the traditional surgery opening hours of 08:30 – 18:30 Monday to Friday.

We are working in partnership across the local area so you may not see a clinician from this practice. The clinician will have access to a summary but not your full notes so appointments are better suited to new conditions or single issue enquiries. The practice will receive a full summary of your consultation and any actions resulting from it the next morning.

Once the benefits of this service have been assessed it may result in longer opening hours being available on a more permanent basis.

Please contact us by phone for details on the locations of the clinics and how to book an evening or weekend appointment.

Text reminder service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

Late for your appointment

Please attend your appointment on time, if you are late you may not be seen. If you are not seen you will not be able to rearrange your appointment until the next working day-except in the event of an medical emergency that requires immediate attention.

Interpreter

If you require an interpreter to attend with you when you see your doctor, speak to a member of the Reception Team and they will arrange this for you.
